What happens to my life insurance when I terminate employment?

When coverage ends nder the plan, you and your dependents can convert your coverages to individual life policies, without evidence of insurability.  The maximum amounts that you can convert are the amounts you and your dependents are insured for under the plan.  You may convert a lower amount of insurance.  Contact the Benefits Office for more information on conversion or portability of life insurance.
