Health Risk Assessment (HRA)

Remember to have your HRA prior to any other preventative services and you will receive an additional $200 towards your wellness benefit.  This brings it up to $700.  The HRA must be done either at St. Johns Hammons Heart Institute or at one of the scheduled school locations.  If you have any questions please contact the office.
